Aging Life Care Manager: Flexible hours
Joan Garbow and Associates, LLC is growing! Are you interested in joining our team? We’re seeking an Aging Life Care Manager to work with our clients throughout Fairfield County, CT. This position is part-time/per diem with potential to grow to full-time.
Responsibilities:
Provide assessments of elderly or disabled clients in their home or facility; formulate and implement short and long-term care plans.
Manage a caseload of clients, provide problem-solving and ongoing care management, and coordination of service providers. Respond to client needs effectively and timely with professional expertise.
Understand and manage complex care needs, attend medical appointments, facilitate medical care.
Family caregiver coaching; communicate with and support family members or legal representatives.
Qualifications:
MSW, RN or other allied healthcare professional.
At least 3-5 years of clinical experience working in a case/care management role with older adults and their families in homecare, acute care, or skilled nursing settings.
Knowledge of:
- the aging process including psychosocial, medical/physical, spiritual, family, and social aspects.
- medical terminology, diagnoses, medications, procedures, hospice.
- healthcare systems, insurance, state and federal programs, various levels of care.
Compassionate, caring professional who focuses on client’s needs, quality of life, and has experience in an advocacy role.
Strong ability to manage multiple tasks; Independent problem-solving skills; Self-motivated individual who works autonomously and efficiently.
Experience working with individuals with dementia, neurologic conditions, complex medical needs, and mental health challenges.
Excellent organizational skills, experience with documentation/billing software, diligent with record keeping.
Follows ethical standards of practice.

Compensation is hourly, including travel, based on experience and certifications.
